How do I prevent warping?

I recently made a stork using 3/8 inch plywood, exterior wood primer and exterior latex house paint. It became extremely warped in less than 2 weeks. How can I prevent this? Also, I have seen a few folks mention "sign board" on the forum and I was wondering what that is. Can you cut and paint it? I searched for "sign board" online and found plastic, wood, and aluminum boards that all are called sign board. Now I am really confused!! Thank you all for the expert advise, Lee (Charleston, SC)
